Hospital discharge support team.
Working alongside other professionals which could include the District Nurse Team, Mental Health Outreach Team, Social Care Team, your GP, and family members to develop and implement a support plan which meets your needs. As well has supporting with your health needs, we’ll support you with your well -being which can be impacted by your environment. We’ll support you to manage your home, plan meals, go shopping and go to social events. The management of your medication is another specialist area of ours. We will support you to order, store and administer your medication. As part of most hospital discharges you will have appointments which we can help you attend by arranging transport and individual support to the appointments.
Behavioural Support
Positive behavioural support (PBS) is a person-centred approach for people who display, or are at risk of displaying, behaviours which challenge. These behaviours always happen for a reason and might be the only way a person can communicate - it can arise for different reasons, many of which are personal to the individual.
